Employment Type:Part timeShift:Description:Employment Type: Part-time .5 (20 hours/week)Shift: DaysDescriptionSupport and advance the mission of service to patients and their families, as part of the Palliative Care team, through the advanced clinical practice of independent professional social work. The primary responsibilities are to coordinate, evaluate, educate, case manage and provide direct care to meet the psychosocial needs of palliative care patients and their families.
