Job Functions: General Responsibilities Residents Rights Restorative Aide Responsibilities Receives assignment from supervisor and performs duties in accordance with each resident’s detailed plan of care to provide care to residents who are presently enrolled in the Restorative Nursing Program. Checks residents in section regularly; Answers call lights promptly; Follows through with resident problems or needs by delivering care or reporting to Supervisor depending upon level of ability and nature of resident need. Maintains knowledge of resident’s condition at all times; Notes significant changes in the condition of residents and takes appropriate action; Identifies emergency situations and responds with appropriate measures. Explains procedures to resident before administering. Takes and records temperature, pulse, respiration, intake and output; Reports resident incidents to immediate supervisor; Communicates requests and problems to appropriate personnel; Takes BPs upon request by Supervisor. Utilizes restorative techniques including cuing, hand over hand, prompting, guarding, etc. Assists in the assessment of resident need for assistive/adaptive devices and communicates those needs to appropriate personnel. Empties urinary drainage bags and records amount in record; Reports any irregularities to nurse. Welcomes new residents and helps them adjust to new environment; Assists residents, family, and staff to ensure a comfortable homelike environment, i.e.: assists resident to personalize room, monitors light and sound levels, temperature, etc. Establishes and maintains good public relations with residents, families, and other center guests; Shows empathy toward residents, as needed. Observes safety needs of residents including use of bedrails, rail pads, and other protective supports when ordered; Identifies safety and security hazards and reports to appropriate personnel after insuring resident is out of immediate danger. Assists resident with personal care, including nails, hair, oral hygiene, and dressing following Universal Precautions Protocols; Encourages resident to do as much as he or she can do him or herself; Cares for resident’s prosthesis as required; Drapes residents for examinations and treatments. Prepares residents for bed rest; Gives special care to residents in casts or traction such as monitoring comfort, skin color, and temperature. Moisturizes resident’s skin; Applies deodorant. Assists residents with their physical needs including eating, bowel, and bladder training, and assistance following incontinence but encourages residents to perform these functions themselves to their capacity or ability; Participates actively in retraining programs for these functions. Follows policies and procedures related to body mechanics, transfer, body positioning, and mobility as outlined in the nursing policies and procedures manual; Repositions residents as needed using supportive devices when necessary (handrails, pillows, etc.) and reports the same. Assists in transferring residents, from chair to bed and back as necessary; Ambulates residents with the use of assistive devices; Assists residents in and out of wheelchairs; Transports residents to treatment units. Prepares resident for meals; Escorts residents to dining room when necessary; Assists with transport of food cart to dietary department; Checks tray with diet card before serving to resident; Sets trays for residents; Serves and collects food trays; Assists residents who require help with the eating process; Charts percentage of meal eaten on food acceptance record; Offers alternate substitute for food not eaten. Delivers between meal nourishment and fresh drinking water to residents, encourages/assists resident in eating/drinking, and documents acceptance. Completes appropriate charge and inventory slips; Monitors supply inventory daily to see what is needed. Cleans instruments and equipment properly and without waste. Labels, separates, and cares for personal care items. Maintains cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms, and linen closets; Cleans bedpans, urinals, and commodes; Washes basins and emesis basins; Sanitizes bath tub after each resident’s use; Cleans floors if resident is incontinent; Wipes spilled foods and liquids. Changes and makes beds; Collects and bags soiled linen and places soiled clothing in appropriate receptacle; Stores clean linen; Places personal laundry in designated place according to center policy; Maintains order of linen cart. Documents residents progress or lack of progress; Assists in resident’s transition from prescribed O.T., P.T., S.T., to restorative program and from restorative program to maintenance by CNA. Participates in conference with Restorative Nurse and Supervisor. Assists in transportation of residents to and from activities. Staff Development Safety and Sanitation Administrative Responsibilities Special Activities/Attributes Education:Successful completion of State Approved Nursing Assistant Training and Competency Evaluation Program; Name must appear on State Registry as Nursing Assistant in good standing. Successful completion of restorative training. CPR certified.Experience:One year experience as Nursing Assistant or Physical Therapist Assistant, preferred.
