Rehabilitation Counselor

We are in search of a self-driven Substance Use Counselor to join our team! We invite applications from CASAC-Ts, CASACs, MHCs, LMSWs, LCSWs, and LMFTs. The ideal candidate will exhibit excellent problem-solving skills, be comfortable working with a diverse population, and possess strong documentation and organizational skills.

Applicants must hold a CASAC-T, CASAC, MHC, LMSW, or LCSW.

Job Details:

The counsellor will be responsible for a caseload of patients in the Program, handling tasks such as intake, orientation, assessment, treatment planning, referrals, individual and group counselling, case management, reporting, record-keeping, and care coordination.

Responsibilities:

• Manage a caseload
• Conduct individual and group counseling sessions weekly
• Perform assessments and intake as necessary
• Develop treatment tracks to meet diverse linguistic and cultural needs of patients and families
• Participate in weekly utilization reviews as per clinical and administrative protocols
• Facilitate monthly skills workshops on chemical dependency
• Review and distribute incident reports according to agency protocol
• Provide input for employee evaluations when requested
• Prepare for weekly case conferences and reviews
• Oversee assessment and intake processes
• Handle the clinical discharge process
• Maintain a daily standard of 10 billable service units based on caseload
• Record patient data in NYC HRA STARS weekly/monthly
• Monitor patient toxicology tests
• Maintain a clean work environment

Schedule:

8-hour or 10-hour shifts 5 days a week including one weekend day
