Design, maintain and evaluate education programs to facilitate management of specific patient populations. Participate in community activities to promote a health and wellness focus. Provide expert content knowledge in field of patient education and programs.Care Coordination: Participates in multi-disciplinary planning of care. Provides consultation in complex breast-feeding situations. Coordinates availability of educational resources. Serves as a link to community resources. Participates in the monitoring of patient/neonatal outcomes.Leadership: Role model positive leadership behaviors. Provide consultation on patient care management and education resources. Support staff in the management of complex patient teaching.Collaborate in identifying opportunities to improve education activities. Participates in the monitoring of patient/neonatal outcomes.Patient Education: Provides input to the RN for establishing a teaching plan. Provides education to the patient / significant others, health care procedures and community at large. Documents patient understanding of education provided.
