This is achieved by coordinating care and service for defined patient populations during an acute care episode across the continuum of care. Case Managers work collaboratively with an interdisciplinary team to improve patient care through the effective utilization of the Health System’s resources. Case Managers are also expected to demonstrate effective Relationship Centered Care and Outcomes Based Care Delivery skills and is accountable for clinical practice and professional leadership.Contingent position scheduled as needed at Trinity Health Ann Arbor. Orientation for this position would last for 6-8 weeks from 8 a.m. to 5 p.m. Monday through Friday.REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSUREEducation: Bachelor's Degree in nursing required.Licensure: Registered Nurse License, State of MichiganExperience: Three (3) years recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager.Certification: Case Management is desirable.Individuals serving as case managers for the health system shall obtain CMSA certification within three years from their date of employment in the role or be in the process of obtaining certification as a case manager.ESSENTIAL FUNCTIONS AND RESPONSIBILITIESThe RN Registered Nurse Case manager is responsible for conducting a focused assessment of patient goals, as well as, biophysical, psychosocial, environmental, economic/financial and discharge planning needs.One would also review, on a concurrent basis, the appropriateness, intensity, and timeliness of the level of care, diagnostic testing and clinical procedures, quality and clinical risk issues and documentation completeness.The RN Registered Nurse Case Manager will develop and document a discharge plan through collaboration with the patient, family and interdisciplinary team.
