As a Registered Nurse - Pediatric Home Care, you will have the unique opportunity to deliver comprehensive nursing care to children with a variety of medical needs, ranging from chronic conditions to acute illnesses. You will work closely with patients and their families to develop and implement personalized care plans, promoting health and wellness while empowering families to manage care at home.

Responsibilities:

• Conduct comprehensive assessments of pediatric patients in their home environment, taking into account their medical, social, and environmental needs.
• Collaborate with patients, families, and interdisciplinary team members to develop individualized care plans that promote optimal health and well-being.
• Provide skilled nursing care, including medication administration, wound care, and infusion therapy, according to physician orders and established protocols.
• Monitor patients' condition and response to treatment, adjusting care plans as needed to achieve desired outcomes.
• Educate patients and families on managing medical conditions, medications, and treatments, empowering them to participate actively in their care.
• Coordinate care with other healthcare providers, community resources, and support services to ensure continuity of care and promote positive outcomes.
• Document all aspects of patient care accurately and in a timely manner, maintaining confidentiality and privacy.
• Advocate for the needs and rights of pediatric patients and their families, ensuring they receive the support and services they need to thrive at home.

Requirements:

• RN/LPN with a valid license to practice as issued by the State Board of Nursing
• Current CPR certification
• Demonstrated passion for working in pediatrics!
