Job Description: Pediatric Home Health RN

As a Pediatric Home Health RN, you will be responsible for delivering high-quality nursing care to pediatric patients in their homes, collaborating with other members of the healthcare team to develop and implement individualized care plans, and providing compassionate support to patients and their families.

Key Responsibilities:

• Provide skilled nursing care to pediatric patients in home settings, including assessment, medication administration, wound care, and other medical procedures as needed.
• Collaborate with physicians, therapists, and other members of the healthcare team to develop comprehensive care plans for pediatric patients, ensuring that each child's unique needs are addressed.
• Educate patients and their families about their medical conditions, treatment options, and preventive care measures, empowering them to actively participate in their healthcare.
• Monitor patient progress, evaluate the effectiveness of interventions, and adjust care plans as necessary to promote optimal health outcomes and ensure continuity of care.
• Document all patient interactions, assessments, interventions, and outcomes accurately and in accordance with regulatory standards and organizational policies.

Requirements:

• RN/LPN with a valid license to practice as issued by the State Board of Nursing
• Current CPR certification
• Demonstrated passion for working in pediatrics!
