Job Description:

We are seeking a dedicated and compassionate Pediatric Home Care Licensed Practical Nurse (LPN) to join our home healthcare team. The Pediatric Home Care LPN will provide essential nursing care to infants, children, and adolescents in their homes. This role requires a blend of strong clinical skills, a deep understanding of pediatric home healthcare needs, and a compassionate approach to providing care to young patients and their families in a home setting.

Key Responsibilities:

1. Patient Care: Deliver high-quality nursing care to pediatric patients in their homes, including routine assessments and treatments in accordance with established protocols and standards.
2. Clinical Assessments: Conduct accurate physical assessments, obtain patient histories, and evaluate patients' overall health to identify any specific healthcare needs in a home setting.
3. Care Planning: Assist in the development, implementation, and evaluation of individualized care plans for pediatric patients, ensuring that all treatments and interventions are age-appropriate and family-centered.
4. Medication Administration: Safely administer medications, monitor for adverse reactions, and educate families on medication management at home.
5. Family Education: Provide education and support to families, ensuring they understand their child's condition, treatment plan, and any necessary home care instructions.
6. Collaboration: Collaborate with RNs, pediatricians, specialists, and other healthcare professionals to coordinate comprehensive care plans for patients.
7. Emergency Response: Assist in responding to pediatric emergencies with prompt and effective care, utilizing basic life support skills.
8. Documentation: Maintain accurate and detailed patient records, including assessments, care plans, interventions, and outcomes.
9. Advocacy: Advocate for the health and well-being of pediatric patients and their families within the healthcare system.
10. Professional Development: Stay current with the latest pediatric home nursing practices, technologies, and research to continually improve the quality of care provided.

Requirements:

• RN/LPN with a valid license to practice as issued by the State Board of Nursing
• Current CPR certification
• Demonstrated passion for working in pediatrics!
