Patient Care Manager

We are committed to providing care and treatment to individuals suffering from substance and alcohol use disorders. We are looking for Patient Care Managers to help clients by reducing treatment barriers and maintaining their engagement in recovery services. Coordinators will model recovery skills and offer hope.

Shifts Available:

• Overnight: Friday, Saturday, Sunday (11:00 pm - 7:00 am)
• Evening: Saturday and Sunday (3:00 pm - 11:00 pm)
• Morning: Saturday and Sunday (7:00 am - 3:00 pm)

Responsibilities:

• Engage with clients in a professional, empathetic manner.
• Communicate with Counselors and Nursing staff regarding client care.
• Foster positive interactions among clients and staff.
• Transport clients within the facility.
• Ensure adherence to the program schedule.
• Conduct room checks.
• Perform census checks every two hours.
• Monitor and report client behaviour.
• Assist in de-escalating challenging situations.
• Oversee client linen and laundry.
• Search and record new admissions' belongings.
• Assist in screening for contraband.
• Act as a role model.

Requirements:

• High school diploma or equivalent.
• One year of related work experience.
