Patient Care Manager

We specialize in the treatment of individuals with substance and alcohol use disorders, focusing on providing the highest quality of care. We are hiring a Patient Care Manager to help patients overcome treatment barriers and stay engaged in recovery services. Coordinators will exemplify recovery skills and inspire hope.

Work Schedule:

• Overnight: 7:00 PM to 7:30 AM, three days a week, including alternating weekends.
• 20 minutes from Lockport

Job Responsibilities:

• Professional, empathetic patient interaction
• Regular communication with counsellors and nursing staff
• Encouraging positive engagement
• Transporting patients within the facility
• Ensuring adherence to activity schedules
• Conducting room and census checks
• Reporting behavioral inconsistencies
• Assisting in de-escalation
• Managing patient laundry
• Screening for contraband
• Performing other assigned duties

Qualifications:

• High school diploma or equivalent
• One year of relevant experience
• Detail-oriented, strong interpersonal and communication skills
• Kind, empathetic, emotionally intelligent
• Able to de-escalate situations and willing to learn
