Patient Care Manager

We specialize in the care and treatment of those with substance and alcohol use disorders, prioritizing patient care above all. We are hiring Patient Care Coordinators who will support patients, reduce treatment barriers, and maintain patient engagement in addiction treatment services. Coordinators will model recovery and coping skills, inspiring hope. Shifts are:

• Morning: 7 am-3:30 pm
• Night: 3 pm-11:30 pm
• Overnight: 11 pm-7:30 am

Responsibilities:

• Interact with patients professionally and empathetically.
• Communicate with Counselors and Nursing staff.
• Encourage positive patient-staff interactions.
• Transport patients within the facility.
• Ensure patients follow the program schedule.
• Conduct room checks.
• Perform census checks every two hours, post-meals included.
• Report behavioural inconsistencies.
• Assist in de-escalating patients.
• Monitor linen and laundry.
• Replenish linen supplies as needed.
• Prepare laundry for housekeeping.
• Return clean laundry to patients.
• Record new admissions' belongings.
• Help screen for contraband.
• Perform other duties as assigned.
• Serve as a role model.

Qualifications:

• High school diploma or equivalent
• One year of related experience Skills:
