Nurse Case Manager

Urban Recovery specializes in treating substance and alcohol use disorders. Our facility provides a full range of care, from detox to medication-assisted treatment, rehab, and recovery therapy. We are currently hiring Nurse Case Manager to join our team!

Overnight shifts: 7 PM - 7:30 AM

The RN role involves coordinating nursing services according to our philosophy and standards, promoting clients' physical and mental health. You'll be part of an interdisciplinary team assisting residents in managing their health and coordinating psychiatric and medication services based on individual needs.

Essential Duties:

• Administer and document medication safely and accurately per PSO policies.
• Conduct urine, blood, and glucose tests.
• Provide direct patient care aligned with the nursing department's goals.
• Assist staff in developing and evaluating care plans.
• Help with care planning, discharge, and continuing care.
• Adjust patient assignments as necessary during shifts.
• Implement patient education programs tailored to specific needs.
• Review and provide feedback on staff documentation.
• Maintain patient care standards per governmental and standardizing agencies.

Requirements:

• Graduate of an accredited nursing school
• Current NY State RN license
• Registered nurse in good standing
• Valid BLS/CPR certification
• Two years of nursing experience
• Preferably experienced in detox/rehab settings
