Nurse Case Manager

Our organization is focused on the care and treatment of individuals with substance and alcohol use disorders. We offer comprehensive services including detox, medication-assisted treatment, rehab, and recovery therapy. We are looking for Registered Nurses (RN) to join our team!

The RN role involves coordinating nursing services in line with our philosophy and standards. This position supports clients' physical and mental health through nursing processes and is part of the interdisciplinary treatment team, helping residents take responsibility for their health. The RN manages medical services, including psychiatric and medication services, according to regulatory and individual needs.

Shifts are 7-7:30.

Duties:

• Administer and document medications safely as per PSO policies.
• Conduct tests for urine, blood, and glucose monitoring.
• Provide direct patient care aligned with nursing department goals.
• Assist in developing and evaluating care plans.
• Support care or discharge planning and continuous care.
• Adjust assignments based on patient needs.
• Implement patient-specific teaching programs.
• Review staff documentation standards and provide feedback.
• Maintain patient care standards in compliance with regulations.

Requirements:

• Graduate from an accredited nursing school.
• Valid RN license in New York State.
• Registered as a professional RN in good standing.
• Current BLS/CPR certification.
• Two years of nursing experience.
• Experience in detox/rehab medication administration.
