Nurse Case Manager

Elev8 Centers focuses on treating individuals with substance and alcohol use disorders. Our facility provides comprehensive care, including detox, medication-assisted treatment, rehab, and recovery therapy. We are seeking Registered Nurses (RN) to join our team!

The RN role involves delivering nursing services per our policies and procedures, promoting clients' physical and mental health. You'll work with an interdisciplinary team to help residents care for their health and coordinate medical services, including psychiatric and medication, based on individual needs.

OVERNIGHT SHIFT 7:00 PM - 7:30 AM

Key Duties:

• Safely and accurately administer and document medications per PSO policies.
• Perform urine, blood, and glucose tests.
• Provide direct patient care consistent with the nursing department's philosophy and goals.
• Assist staff in developing goal-oriented care plans and evaluating care.
• Help plan patient care, discharge, and ongoing care.
• Adjust patient assignments based on changing needs during shifts.
• Implement patient education programs based on specific needs.
• Review and provide feedback on staff documentation standards.
• Maintain patient care standards as per government and standardizing agency regulations.

Minimum Qualifications:

• Graduate of an accredited nursing school
• Current New York State RN license
• Registered nurse in good standing
• Valid BLS/CPR certification
• Two years of nursing experience
• Experience in medication administration in a detox/rehab setting
