Job Description: Medication Specialist

We are currently seeking a knowledgeable and detail-oriented Medication Specialist to join our healthcare team. As a Medication Specialist, you will play a critical role in ensuring the safe and effective use of medications, providing expertise in medication management, and promoting optimal health outcomes for patients. Responsibilities:

  1. Medication Review: Conduct comprehensive reviews of patient medication profiles to assess appropriateness, safety, and adherence to treatment guidelines. Identify potential drug interactions, duplications, contraindications, and adverse effects, and collaborate with healthcare providers to optimize medication regimens.

  2. Medication Counseling: Provide patient education and counseling on medication use, including dosage instructions, administration techniques, potential side effects, and drug interactions. Address patient questions and concerns, and empower patients to actively participate in their medication management and treatment decisions.

  3. Medication Reconciliation: Perform medication reconciliation processes to ensure accurate and complete medication histories for patients across care transitions, including hospital admissions, discharges, and transfers. Communicate with patients, caregivers, and healthcare providers to reconcile discrepancies and prevent medication errors.

  4. Medication Management: Oversee medication management processes, including prescription refills, prior authorizations, and medication ordering and dispensing. Collaborate with pharmacists, physicians, and other healthcare providers to facilitate timely access to medications and ensure continuity of care.

  5. Quality Improvement: Participate in quality improvement initiatives related to medication safety, medication management, and adherence to evidence-based prescribing practices. Collect and analyze data, identify opportunities for improvement, and implement interventions to enhance medication-related outcomes and patient safety.

  6. Clinical Consultation: Provide clinical consultation and support to healthcare providers, including physicians, nurses, and allied health professionals, regarding medication-related issues and therapeutic recommendations. Serve as a resource for drug information and evidence-based practice guidelines.

  7. Patient Advocacy: Advocate for patient safety and quality of care by identifying and addressing medication-related risks, errors, and adverse events. Collaborate with interdisciplinary teams to develop and implement strategies to mitigate risks and improve medication-related processes and outcomes.

Requirements:

  1. Education: Bachelor's degree in Pharmacy, Pharmacology, or related field required. Doctor of Pharmacy (PharmD) degree preferred.

  2. Licensure/Certification: Current licensure as a Pharmacist (RPh). Additional certifications in medication therapy management or pharmacotherapy may be preferred.

  3. Experience: Previous experience in medication management, clinical pharmacy, or medication therapy management preferred. Familiarity with medication reconciliation processes, drug information resources, and evidence-based prescribing practices.

  4. Interpersonal Skills: Excellent communication, counseling, and interpersonal skills. Ability to effectively communicate complex medication information to patients, caregivers, and healthcare providers.

  5. Analytical Skills: Strong analytical and critical thinking skills. Ability to interpret medical and pharmaceutical information, analyze medication-related data, and develop evidence-based recommendations.
