Short Description

Manager, Nurse Navigator, Ambulatory Operations shall support registered nurses and LPNs within Ambulatory Operations Patient Experience Center to provide important leadership and evidence based clinical guidance for work that relates to clinical staff in ambulatory care.

Supervises and directs the Ambulatory Operations Patient Experience Center LPNs and RN Navigators.

Supervises and directs navigator support, education, and assistance to patients and their families.

This role is key in the development and implementation of multi-disciplinary care.

Follows Standards and Recommended Practices and other professional standards.

Evaluates and interprets policies and objectives ensure compliance and consistent quality of care.

Ensures physicians are assisted in clinical operations through provision of an adequate, qualified staff.

Plans assignments for personnel and delegates specific duties and tasks.

Evaluates needs for new or different items regarding department needs and current trends.

Provides educational and technical opportunities for all staff members. Directs program of in-service education regarding objectives, content, and need for subject matter.

Develop a metric system that monitors navigation quality metrics for both nurse navigator and LPNs.

Develop core elements of practice for nurse and patient navigation in order to define expectations for patients and providers

Ensures the completeness of all related documentation in a timely manner; provides communication throughout the team on an on-going basis.

Understands the demands and experiences of patient care, the disease process/treatment and anticipates patient and family needs.

Triages/forwards questions to the appropriate disciplines

Works efficiently, is flexible and sets priorities.

Contributes to the oversight of patient care operations and assuring staffing schedules to meet patient needs in direct patient care, communication with patients and navigation to care.

Partners with providers in specialty to address clinical needs for direct patient care and communication.

Ensures the delivery of quality patient care via management of clinical services, including clinical orientation and clinical competency, of non-physician providers, including nursing staff.

Partners with physicians, clinical personnel and administrative leadership within their assigned areas of the health system to remove barriers to success and achieve local and organizational goals.

Provides strong, effective, goal-oriented leadership to direct and indirect reports

Meets goals, objectives, policies, procedures and systems for all operational areas within span of control.

Develops, coordinates, and maintains standardized work procedures and policies to improve efficiency and effectiveness across Ambulatory Operations.

Coaches, develops, educates, mentors, and holds accountable direct reports as well as fosters an environment of trust throughout their areas of responsibility.

Hardwires and validates leadership tools such as leader rounding on patients, employees, and providers and practices reward and recognize of key behaviors

Participates in professional development activities and maintain professional affiliations.

Performs all related duties and/or special projects as assigned/required.
