Employment Type:Full timeShift:Day ShiftDescription:Reporting to the Manager, Care Coordination and Social Services, Social Services, this position is responsible for assessing the psychosocial needs of patients and families and providing intervention to meet those needs utilizing personal, family, and community resources.
