Job Description: In-Pediatric Home Health RN

Are you a dedicated Registered Nurse (RN) with a focus on pediatric home health care? We are seeking a compassionate and experienced In-Pediatric Home Health RN to join our team. As an In-Pediatric Home Health RN, you will provide high-quality nursing care to pediatric patients in their homes, ensuring they receive the best possible care in a familiar and comfortable environment.

Key Responsibilities:

• Patient Care: Deliver high-quality nursing care to pediatric patients in their homes, addressing their physical, emotional, and developmental needs. Administer medications, treatments, and procedures as prescribed.
• Assessment and Monitoring: Perform thorough assessments of patients' conditions, monitor vital signs, and document changes. As an In-Pediatric Home Health RN, you will ensure early detection of potential health issues.
• Care Planning: Develop and implement individualized care plans in collaboration with the healthcare team. Adjust plans based on patient progress and clinical guidelines.
• Family Support: Educate and support families, providing them with the information and resources needed to care for their children. Foster a supportive environment that encourages family involvement.
• Coordination of Care: Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated and comprehensive care for patients. Participate in multidisciplinary team meetings.
• Documentation: Maintain accurate and detailed patient records, ensuring all documentation meets regulatory and professional standards.
• Professional Development: Stay current with the latest developments in pediatric home health nursing through continuing education and professional development. Enhance skills to provide the best care possible.

Requirements:

• RN/LPN with a valid license to practice as issued by the State Board of Nursing
• Current CPR certification
• Demonstrated passion for working in pediatrics!
