Hospice Social Worker

At Surfpoint Recovery, our inpatient drug treatment program fosters career growth for clinical professionals. Collaborate with a team of clinicians, nurses, providers, and peer advocates to develop comprehensive client care plans.

The LCSW will provide supervision to LMSWs, supporting their professional development and enhancing leadership and mentoring abilities.

This position may be a combination of in-person and remote work.

Responsibilities:

• Overseeing a caseload of 8-10 clients
• Performing daily individual and group counseling sessions
• Offering aftercare planning, crisis intervention, and developing personalized treatment plans
• Maintaining compliance with documentation and progress notes
• Following OASAS standards
• Helping with weekly utilization reviews according to clinical and administrative guidelines
• Contributing to the preparation of weekly case conferences and reviews
• Supervising MSW student interns as needed

Requirements:

Master's degree required Valid New York State Licensed Clinical Social Worker in good standing SIFI preferred, or willingness to complete a SIFI course At least 3 years of relevant experience.
