Home Health Nurse

Our organization is dedicated to the treatment and care of individuals with substance and alcohol use disorders. We offer a comprehensive range of services, including detox, medication-assisted treatment, rehab, and recovery therapy. We are looking for Registered Nurses (RN) to join our team!

The RN role involves coordinating nursing services in accordance with our philosophy and standards. This position supports clients' physical and mental health through nursing processes and is part of the interdisciplinary treatment team, helping residents take responsibility for their health. The RN oversees medical services, including psychiatric and medication services, according to regulatory and individual needs.

Shifts are from 7 to 7:30.

Essential Duties:

• Safely administer and document medications per PSO policies.
• Conduct urine, blood, and glucose monitoring tests.
• Provide direct patient care aligned with nursing department goals.
• Develop and evaluate care plans.
• Assist in care or discharge planning and continuous care.
• Adjust assignments based on patient needs.
• Implement patient-specific teaching programs.
• Review staff documentation standards and provide feedback.
• Maintain patient care standards as per regulations.

Qualifications:

• Graduate from an accredited nursing school.
• Valid RN license in New York State.
• Registered as a professional RN in good standing.
• Current BLS/CPR certification.
• Two years of nursing experience.
• Experience in detox/rehab medication administration.
