Health Care Coordinator

We provide care and treatment for those with substance and alcohol use disorders, focusing on patient care. We are hiring Health Care Coordinators to support patients, reduce barriers, and keep them engaged in treatment. Coordinators will model recovery skills and offer hope. Shifts:

• Morning: 7 am-3:30 pm
• Night: 3 pm-11:30 pm
• Overnight: 11 pm-7:30 am

Duties:

• Interact with patients professionally and empathetically.
• Communicate with Counselors and Nursing staff.
• Promote positive interactions.
• Transport patients.
• Ensure adherence to schedules.
• Conduct room checks.
• Perform census checks every two hours.
• Report behavior inconsistencies.
• De-escalate patients.
• Oversee linen and laundry.
• Replenish linen supplies.
• Prepare laundry for housekeeping.
• Deliver clean laundry.
• Document new admissions' belongings.
• Screen for contraband.
• Perform assigned duties.
• Be a role model.

Requirements:

• High school diploma or equivalent
• One year of related experience Skills:
