We are looking for a compassionate and experienced Health and Wellness Nurse (Pediatric Home Care) to join our team. As a Health and Wellness Nurse (Pediatric Home Care), you will provide comprehensive health and wellness services to pediatric patients in their homes, ensuring they receive the highest standard of care in a familiar environment.

Key Responsibilities:

• Preventative Care: Deliver preventative healthcare services, including vaccinations, health screenings, and wellness checks, to pediatric patients in their homes.
• Health Assessments: Conduct thorough health assessments to identify potential health issues and developmental concerns. Monitor growth and development milestones.
• Care Planning: Develop and implement individualized wellness plans tailored to each child's specific needs. Collaborate with families and healthcare professionals to ensure holistic care.
• Health Education: Educate children and their families on healthy lifestyle choices, nutrition, hygiene, and disease prevention. Empower families with knowledge and resources.
• Chronic Condition Management: Assist in the management of chronic conditions such as asthma, diabetes, and allergies, ensuring adherence to treatment plans and medications.
• Coordination of Care: Work closely with pediatricians, specialists, and other healthcare providers to coordinate comprehensive care. Participate in interdisciplinary team meetings.
• Documentation: Maintain accurate and detailed patient records, ensuring all documentation meets professional and regulatory standards.
• Professional Development: Stay current with the latest advancements in pediatric health and wellness through continuing education and professional development.

Requirements:

• RN/LPN with a valid license to practice as issued by the State Board of Nursing
• Current CPR certification
• Demonstrated passion for working in pediatrics!
