Employment Type:Full timeShift:Evening ShiftDescription:Full-Time On-Call Hospice Visit NurseThe Community Hospice has a wonderful opportunity for a FT on call/ visit RN to work EVENINGS as part of our Hospice Care Team caring for patients and families serving Capitol Region. At times, when necessary-might be asked to assist with patients in Saratoga/Washington/Montgomery/Columbia and Greene Counties.Hours are: 4pm-Midnight and includes an every other weekend rotationPosition Highlights:National pre-taxed mileage reimbursementMeetings and charting conducted via supplied lap topStipend for cell phone is providedComprehensive orientation providedFull benefits through St Peters Health partnersPrioritizing Your Safety: Ensuring your safety is our top priority. From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.What you will do:As an RN you will visit patients in the home setting and local nursing homes to respond to symptom management needsCoordinate with an interdisciplinary team to provide added support to patients and families in their homes.This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).What you will need:NYS RN LicensureAssociates Degree in Nursing, BSN preferred1-2 years RN experience required-preferably in homecare or acute carePrevious hospice experience preferred but NOT required.The RN must be able to communicate effectively, verbally and writtenValid Driver's License and Reliable vehicleHigh level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers​Community Hospice provides a thorough orientation, preceptors and a supportive work environment.Apply today for more information.
