PURPOSE:Assist in providing all indirect, non-personal care to all residents.Accountable for reporting any changes in resident condition and following the practices and requirements of the policies and procedures of this facility in accordance with current federal, state, local standards governing the facility.
QUALIFICATIONS:
Prior experience long-term care experience is desirable.
Sufficient education to demonstrate functional literacy.
DUTIES:
·Report all pertinent information concerning resident care as detected to appropriate supervisor/personnel.
·Report requests of care needs to appropriate staff.
·Transport resident to activities, therapy, outside to smoke, etc.
·Makes beds, distributes linen as needed, transport dirty linen to linen room using established standard precautions.
·Distributes and sets up food trays for residents during meal times, collect empty meal trays from resident rooms and dining rooms. Monitor dining areas as assigned.
·Passes water pitchers to residents and ensures that cup is within reach of resident, replaces straw in cup for resident.
·Clean and organize residents’ dresser drawers, closets and refrigerators as needed.
·Label resident personal belongings
·Ensure call bells and bed remotes are untangled and in reach of resident
·Clean brushes and combs for residents.
·May apply nail polish if resident desires.
·Restock supplies.
·Answer call lights and obtain assigned staff member if direct patient care is needed.
·Complete assignments timely, completely and accurately
·Conduct resident rounds as assigned
·Identify resident problems and concerns and report them immediately to a charge nurse or licensed nurse.
·Initiate corrective action as necessary and/or seek assistance of a licensed nurse
·Identify safety hazards and emergency situations and initiate corrective action.
·Attend all in-service classes as assigned and complete assignments
DUTIES (Continued):
·Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit
·Document incidents and/or unusual problems according to established facility procedures
·Participate in interdisciplinary team and other facility meetings as assigned
·Document care and treatment provided to residents and the resident's response or lack of response to the treatment provided
·Maintain confidentiality of resident and facility information
·Follow facility procedure for admission, discharge and transfer of residents
·Assure residents have call lights at hand and answer call lights promptly
·Assure residents are given adequate assistance with meals, nourishment and transportation to meal service areas
·Come to work in a clean, neat uniform and consistently present an appropriate professional appearance
·Come to work as scheduled and consistently demonstrate dependability and punctuality
·Assume accountability for data contained in the employee handbook
·Additional duties as assigned by the supervisor
