Job Description:
- Develops and presents health education and promotion programs, including training and providing literature for adolescent transition to adult care.
- Assists adolescents with understanding their care and personal development including personal and social relationships.
- Calls adolescents with appointment reminders and follows up with available extended services in the community.
- Works collaboratively with community partners, and members of the client's treatment team. Maintains all records and reports related to the adolescent program and (adolescent) patient enrollment., Keeps track of data in an Excel spreadsheet.
- Coordinates bi-monthly adolescent committee, serves on health- related task forces and advisory committees, including the- Clinical Effectiveness Group (CEG).
- Provides case management services for assistance with any additional needs outside the scope. Performs HIV Testing if necessary.
Expected Salary:
$39,700 to $46,000 annually depending on education and experience.
Minimum Requirements:
Bachelor's degree in an appropriate; or an equivalent combination of education and experience.
Preferred Qualifications:
- HIV 501 class
- 6 months on site training or 1 year experience with HIV population
- Adult/Pediatrics CPR certification preferred
