Clinical Care Coordinator

We focus on caring for those with substance and alcohol use disorders, always putting patient care first. We are looking for Clinical Care Coordinators to support patients by reducing treatment barriers and keeping them engaged in addiction recovery. Coordinators will model recovery skills and inspire hope.

Shift Details:

• Overnight: 7:00 PM to 7:30 AM, three days weekly, with alternating weekends.
• Location: 20 minutes from Lockport

Responsibilities:

• Professional and empathetic patient interaction
• Coordination with counsellors and nurses
• Encouraging positive engagement
• Transporting patients within the facility
• Ensuring schedule adherence
• Conducting room and census checks
• Reporting behavioral inconsistencies
• Assisting in de-escalation
• Managing patient laundry
• Screening for contraband
• Additional duties as needed

Requirements:

• High school diploma or equivalent
• One year of relevant work experience
• Detail-oriented with excellent interpersonal and communication skills
• Kindness, empathy, emotional intelligence
• Ability to handle hostile situations and eagerness to learn
