The care coordination supervisor is responsible for the clinical and administrative oversight of all assigned care coordination staff. The care coordination supervisor is also responsible for assisting in recruitment, hiring, and training of new care coordination staff as well as providing ongoing coaching and clinical development of assigned care coordinators. care coordinators provide a wide variety of services that focus on the mental health needs of the client. These include client skills training, parent skills training, community resource connection, and treatment team leadership and coordination. The care coordination supervisor should be comfortable and familiar with coaching new staff and implementing services as needed in those four areas. The care coordination supervisor must be committed to serving clients from diverse backgrounds and promoting equity and inclusion in service delivery.

Primary Responsibilities:

Supervision

  • Responsible for the day-to-day oversight of services provided by direct reports in home, school and community settings.
  • Provides clinical supervision and direction for all staff as assigned, providing weekly individual supervision according to OMHAS, CARF, and agency guidelines. Responsible for being available to provide support and intervention, as needed. Promotes excellence in clinical practice. Ensures a culture of ethical practice, integrity, and a positive work climate.
  • Participates in interviewing, selecting, training, evaluating, and professionally developing care coordination staff.
  • Provides new employee orientation, onsite consultation and ongoing training as needed for clinical competence, agency compliance to standards of care within the community, and regulatory guidelines related to clinical practice.

Administration

  • Provides administrative supervision and direction for assigned staff. Ensures compliance with all service, administrative and operational standards and regulations for program area. Reviews client charts of direct reports at regularly scheduled supervisory intervals to ensure compliance with program regulatory standards (including OMHAS and CARF) and financial auditing requirements. Maintains a working knowledge of all general agency policy and procedures as outlined in the agency supervisory manual.
  • Ensures the timely and accurate submission of all documentation as required of direct service providers. Oversees providers and tracks their administrative responsibilities to ensure that they are carrying out responsibilities efficiently and effectively. Assists direct reports with time management, caseload management and with meeting all administrative expectations.
  • Ensures that direct service staff participates in achieving organizational mission through knowledge of agency, departmental, and programmatic outcomes and works on an individual and group level toward meeting those outcomes.

Internal and External Collaboration

  • Works collaboratively and maintains professional relationships within the program area, department and across the agency. Collaborates with others to ensure operational quality, efficiency and effectiveness. Promotes effective communication.
  • Promotes, ensures, and models customer service and a public relations perspective at all times both within the department, organization as well as within the community at large.
  • Ensures that direct service providers consult, collaborate, and coordinate treatment with other provider agencies and systems on behalf of clients and families. Ensures direct reports maintain consistent and constructive communication with all members of the treatment team, both internal and external to the agency.

Direct Client Service

  • Provides assessment, BHC and/or care coordination services to clients as needed (e.g. supporting staff caseloads, new intakes, etc). Evaluates progress of each client at required intervals. Delivers service to other individuals who may assist in that client’s mental health treatment. Develops a transition plan with staff for each client being discharged from service and ensures it is carried out prior to closing.

Quality Assurance and Effectiveness

  • Ensures effective provider performance and quality service delivery based on best practice models. Promotes family based treatment models.
  • Ensures quality within program area and identifies appropriate actions to accurately reflect and promote resolution of any identified challenges. Ensures direct reports are aware of and work toward meeting quality indicators. Works with program manager in defining and implementing performance improvement activities and action plans as required.
  • Ensures direct service staff attends agency trainings or professional seminars/conferences as required or necessary in order to maintain compliance with professional licensure and to increase professional knowledge and skill base.

Fiscal Responsibility

  • Assists the Program Manager in the development of annual program budgets. Demonstrates an understanding of program and departmental fiscal targets and responsibilities. Assists program manager in ensuring that monthly and annual budgetary targets, for both revenue and expenses, are met or exceeded.
  • Ensures that direct service staff understands programmatic and departmental fiscal targets and responsibilities, and demonstrate understanding and execution of sound business practices (e.g. revenue generation and expense control). Actively assists direct reports in understanding the budgetary process.
  • Works closely with staff to ensure the meeting or exceeding of monthly and annual productivity targets. Actively assists direct reports to manage caseload size and case acuity to ensure that the numbers of clients scheduled weekly is sufficient to meet or exceed required productivity.
  • Assists program manager in program development and expansion activities.

Other duties as assigned

Minimum Education, Experience and Other Skill Requirements:

This position requires a Master’s degree from an accredited college or university in Social Work, Counseling, Psychology or a related field. Three years of post-graduate experience and State of Ohio Licensure (LSW, LPC, MFT) required, Independent Licensure preferred (LISW, LPCC, IMFT or Licensed Psychologist). Clinical Supervisory experience strongly preferred. A valid State of Ohio driver’s license, a good driving record, automobile insurance and the use of a reliable automobile is required.

Must possess minimum technical skills including familiarity with Microsoft Windows and ability to use standard Microsoft Office applications.

Work Environment:

Exposed to a combination of office, school and mental health treatment environments. Exposed to children with behavior problems. May be exposed to shouting and clients who may be verbally and physically aggressive. May be exposed to blood or vomit and bodily fluids. May be exposed to unclean environments and unfamiliar areas of town.

Regularly required to sit, stand, walk, bend and lift up to 50 pounds. Ability to operate a motor vehicle safely in changing weather conditions required.
