Assist in assessing residents'/patients' social and emotional needs and developing individualized care plans in collaboration with the interdisciplinary team. Maintain accurate and up-to-date documentation of social work assessments, interventions, and progress notes in accordance with regulatory standards. Assist residents/patients and their families in accessing community resources, support services, and financial assistance programs as needed. Advocate for residents'/patients' rights and interests and collaborate with healthcare teams to ensure their psychosocial needs are addressed effectively. Provide education and support to families on coping strategies, communication skills, and caregiving responsibilities. Assist in managing crisis situations and providing emotional support and crisis intervention services to residents/patients and families during challenging times. Collaborate with healthcare professionals, including nurses, physicians, therapists, and administrators, to coordinate care and address psychosocial issues impacting residents'/patients' well-being. Participate in ongoing training and professional development opportunities to enhance knowledge and skills in social work practice and geriatric care.:Assistant Social Worker Qualifications: Bachelors Degree in social work or related field. Previous experience in a social work or related role preferred but not required. Strong communication and interpersonal skills, with the ability to build rapport and establish trust with residents/patients and families. Compassion, empathy, and a patient-centered approach to care. Knowledge of social work principles, ethics, and practices. Ability to work independently and as part of a team in a fast-paced environment. Proficiency in computer applications and electronic health records systems..
