The ideal applicant will demonstrate strong problem-solving skills, the ability to work with a diverse population, and excellent documentation and organizational capabilities.
Applicants must possess a CASAC-T, CASAC, MHC, LMSW, or LCSW.
Job Description:
The counsellor will oversee a caseload of patients in the Program. Responsibilities include intake, orientation, assessment, treatment planning, referrals, individual and group counselling, case management, reporting, record-keeping, and care coordination.
Duties:
- Manage a caseload
- Conduct weekly individual and group counseling sessions
- Perform assessments and intakes as necessary
- Develop treatment tracks that address diverse language and cultural needs of patients and families
- Participate in weekly utilization reviews according to clinical and administrative protocols
- Facilitate monthly workshops on chemical dependency
- Review, process, and distribute incident reports as per agency protocol
- Provide input for employee evaluations when requested
- Prepare for weekly case conferences and reviews
- Handle the assessment and intake process
- Oversee the clinical discharge process
- Maintain a standard of 10 billable service units per day based on caseload
- Record patient data in NYC HRA STARS weekly/monthly
- Monitor patient toxicology tests
- Maintain a clean work environment
